Medical Form FAQs

A medical form needs to filled out for all cub scouts so they can participate in outings such as field trips and camping trips.
Notes:
Parts A and C are to be completed annually by all BSA unit members.  (This means Scouts and Leaders)

Part C includes the parental informed consent, the hold harmless/release agreement and a talent release statement.
Part B is only required with parts A and C for any event that exceeds 72 consecutive hours, a resident camp setting, or when the nature of the activity is strenuous and demanding, such as service projects, work weekends, or high-adventure treks.
Part B should be completed by all Leaders.
